Case Management

This service continues to be vital to our community and is available to those who are either not eligible for services or supports through the National Disability Insurance Scheme (NDIS) or do not have an NDIS plan.

This program is state-wide and continues to have a person-centred approach. Our case managers work together with their client to achieve identifiable and achievable goals, predominantly in relation to regaining control over their lives.

The goals that our case managers assist our clients to achieve, all impact on their health and wellbeing and their ability to attain the highest quality of life whilst living with HD. An example of some of the goal areas are listed below:

  • Disease and symptom management
  • Social and emotional wellbeing
  • Financial security
  • Housing stability

Our case management program remains a voluntary service, meaning everyone who participates must be able to provide informed consent, or have their representative (NOK, legal decision maker) provide that consent on their behalf.
