Community Care services

Community Care Services are planned and coordinated packages of care. These services aim to help people with a disability live independently in their own homes and enjoy quality of life for as long as possible. Case Managers work in partnership with clients and their families and carers to identify needs and develop a support plan to increase the client's capacity and family's resilience.

As Huntington’s disease is a degenerative condition, support needs can change quickly. It’s important to plan for each stage of caring. Your local Case Manager (see Linkages below) can help you with detailed planning.

Community Care Services should:

  • Focus on the individual.
  • Respect  the person’s values, preferences and expressed needs
  • Allow informed and shared decision-making
  • Meet the person’s mental health needs as well as physical.
